LifeWise Health Plan of Washington

• Available to residents of all 39 Washington State counties
• All plans are traditional or Preferred Provider (PPO) plans, so no Primary Care Physician (PCP) is needed and no referrals are required
• Largest choice of providers
• Varying deductibles offered
• HSA (Health Savings Account) plans available with varying individual and family deductibles
• Preventive Care covered at 100%, not subject to the deductible
• Prescription drugs may be covered (subject to a separate deductible)
• Maternity is an option in some plans
• Worldwide coverage with advantages for using Blue Shield / Blue Cross Providers

Regence BlueShield

• Available to residents of 22 Washington State counties.
• PPO and Point of Service (POS) plans
• Large choice of providers
• Varying deductibles
• Preventive Care covered at 100%, not subject to the deductible
• Some programs offer prescription drug coverage, and others may offer a discount program
• Maternity may or may not be covered, depending on the plan selected
• Worldwide coverage

Group Health Cooperative

• Available to residents of 18 Washington State counties.
• Most popular plan allows the insured to use either the Group Health network or provider of their choice
• Variable deductibles
• 70-80% coverage
• Preventive Care covered in all plans at 100% (not subject to the deductible)
• Prescription drugs may or may not be covered in all plans
• Maternity may or may not be covered
• Worldwide coverage

Asuris Northwest Health

• Available to residents of 16 eastern Washington State counties.
• PPO plans
• Large choice of providers
• Variable deductibles
• 80% coverage
• Preventive Care covered at 100%, not subject to the deductible with most plans
• Prescription drugs may be covered in some plans, discount programs are available for others
• Maternity is an option in some plans
• Worldwide coverage

KPS Health Plans

• Available to residents of 8 western Washington State counties.
• All plans are PPO plans
• Large choice of providers
• Variable deductibles
• 80% coverage
• Preventive Care is usually covered in full, may be a small copay
• Prescription drugs covered in $500 deductible plan; other plans have discounts available at participating pharmacies
• Maternity may or may not be covered
• Worldwide coverage
